Trivia questions : What is terbutaline?

    Terbutaline mechanism of action

It has high selectivity for airway β2 receptors, and its bronchodilator effect is similar to that of salbutamol. The effect on the heart is only 1/100 of isoproterenol. This product replaces the catechol ring with a m-hydroxyphenol ring, and the hydrogen atom of the amino group is replaced by a tert-butyl group on the side chain of ethanolamine. The structural change makes it difficult to be inactivated by COMT, monoamine oxidase or sulfate kinase, and the action time is significantly prolonged, which has become its most significant feature. This product can also excite the β2 receptors in the myometrium and inhibit spontaneous contraction of the uterus or uterine contractions caused by oxytocin.

    Terbutaline synthesis

3,5-dihydroxybenzoic acid and ethanol are reacted under reflux under sulfuric acid catalysis to obtain ethyl 3,5-dihydroxybenzoate. Ethyl 3,5-dihydroxybenzoate and benzyl chloride in the presence of potassium carbonate, refluxed for 20 h, hydrolyzed with 5 ml / L of hydrochloric acid or potassium hydroxide to the free acid, and refluxed with sulfoxide for 1 h, and chlorinated to The acid chloride reacted with azomethane to form azoacetyl, and reacted with hydrogen bromide to form bromoacetyl, and then refluxed with tert-butylamine for 20h. Terbutaline was finally obtained by catalytic hydrogenation in the presence of acetic acid.

    What is terbutaline sulfate

    Terbutaline sulfate

This product is an adrenergic agonist. It can selectively activate β2-receptors, relax bronchial smooth muscle, inhibit the release of endogenous spastic substances and edema caused by endogenous mediators, improve the ability of bronchial mucosal ciliary epithelium to clear, and relax uterine smooth muscle.

    Terbutaline sulfate interactions

    Terbutaline sulfate interactions
1 Concomitant use of other adrenergic receptor agonists can increase efficacy, but adverse reactions may also worsen.
2 Concomitant use of theophylline drugs can increase efficacy, but palpitations and other adverse reactions may also worsen.
3 Non-selective β-blockers can partially or completely inhibit the effects of the drug.

    Terbutaline uses

    Terbutaline for asthma

Drugs used to treat bronchial asthma, asthmatic bronchitis, emphysema, etc. β2-receptor stimulant with bronchiectasis. It is highly selective for bronchial smooth muscle, has little excitatory effect on the heart, and has no central effect. For bronchial asthma, asthmatic bronchitis, obstructive emphysema, and other lung diseases with bronchospasm. It is especially suitable for asthma patients with cardiovascular diseases such as hypertension and coronary heart disease.

    Terbutaline for preterm labor

Doctors may try to stop or delay preterm labor by administering a medication called terbutaline. Terbutaline is in a class of drugs called betamimetics. They help prevent and slow contractions of the uterus. It may help delay birth for several hours or days. During that period, doctors can administer other drugs to help ensure the baby is born as healthy as possible. One of those drugs is given to the mother to help the baby’s lungs mature faster. These drugs need 12 to 72 hours to begin to work. Using terbutaline delays delivery for several days (at least) and allows time for the drugs to work.

    Terbutaline for priapism

Little is known about the mechanism of terbutaline in erections. Although it is a beta-2 receptor agonist and smooth muscle relaxant, abnormal penile blood stagnation prevents terbutaline from perfusing into the cavernous body, suggesting other alternatives, if any.

    Terbutaline dosage

    1. Oral: 2.5 to 5 mg per adult, 3 times a day. Child reduction. 2. Aerosol inhalation: 0.2 mg per adult, 3 to 4 times per day. 3. Subcutaneous injection: 0.25 mg each time, if there is no obvious clinical improvement in 15-30 minutes, the injection can be repeated once, but the total amount in 4h cannot exceed 0.5 mg.

    Terbutaline adverse effects

    Terbutaline interactions

    1. The combination of this drug with other adrenergic receptor agonists can increase the efficacy, but the adverse reactions may also worsen.
    2. Monoamine oxidase inhibitors, tricyclic antidepressants, antihistamines, levothyroxine, etc. can enhance the adverse reactions of this drug.
    3. This medicine combined with succinylcholine can enhance the muscle relaxation of the latter.
    4. This medicine can reduce the blood pressure-lowering effect of guanethidine.
    5. This medicine can enhance the adverse reactions of non-potassium-preserving diuretics.
    6. Epinephrine beta blockers (such as acebutolol, atenolol, labelol, metoprolol, naldolol, indolol, propranolol, timolol Seoul, etc.) can antagonize the effect of this drug and reduce the efficacy.
    7. When this medicine is used in combination with insulin or oral hypoglycemic agents, the dosage of the latter must be adjusted.
    8. When this medicine is combined with theophylline. Can reduce theophylline blood concentration.

    Terbutaline indications

For bronchospasm in bronchial asthma, asthmatic bronchitis, and chronic obstructive pulmonary disease. Continuous intravenous infusion of this product can stimulate uterine smooth muscle β2 receptor, inhibit spontaneous uterine contraction and uterine contraction caused by oxytocin, and prevent premature birth. The same principle can be applied to fetal asphyxia.

Terbutaline is also used as an additive in feed, which can significantly improve animal performance, improve feed-to-meat ratio, and increase lean meat rate. However, research has found that terbutaline residues in animal-derived foods can cause great harm to humans, so many countries or organizations in the world have banned them as feed additives. In order to save costs and obtain greater profits, some illegal producers often illegally use illegal additives that have proven to be harmful to the human body and are prohibited by the state's regulations, which brings great hidden danger to people's health.
The Terbutaline Rapid Test (Urine) kit provided by BALLYA can effectively detect the presence of terbutaline in food. This kit is not only simple to operate, but also has a short test time and high accuracy. Can ensure consumers Food safety.
